Obtaining FHL Film Numbers for NYC Vital Records in One Step

WHEN TO USE THIS FORM
There are several forms on my website for obtaining certificate numbers for New York City vital records. Specifically:
births
marriages
deaths
Once you know the certificate number, you can view the microfilm roll containing the actual certificate for free at the Family History Library (FHL) in Salt Lake City. If you are not in Salt Lake City, for a small fee you can order the microfilm or order a printout of a specific certificate on the microfilm from any of their Family History Centers around the world.
But in order to do any of the above, you need to know the FHL film number. This form lets you determine the FHL film number if you know the certificate number. It also lets you determine the film number of the roll containing the index to the certificate, which in turn will give you the certificate number.
